Developmental Pathways is seeking a
Family Support Coordinator who will coordinate support services for adults and/or children living in a variety of settings. Our Family Support Services Coordinator will communicate with a broad team of providers including professionals, direct care professional employees, families, and natural supports chosen by the individual or family being served. You’ll also get to assist with our department’s initiatives and outreach efforts.

This is a hybrid position, working remotely and coming into our headquarters office in Aurora, CO and/or off-site locations, as needed. This is a full-time position at 40-hours/week.
More about what you’ll get to do as our Family Support Coordinator
  • Research, request, obtain potential services and/or providers to meet individual needs/ preferences as determined in the Family Support Plan (FSP) and/or Community Outreach Enrollment Form
  • Mentor individual’s service choices; help families determine priorities for service
  • Respond promptly and professionally to individual/family needs and requests
  • Develop and review FSP with family and multidisciplinary team where applicable
  • Prepare family for transitions to other services as appropriate
  • Attend and participate in meetings; schedule and facilitate as needed
  • Assist with initiatives, meeting outreach goals and with events such as Community Outreach Resource Fair, Holiday Giving Projects, Parent Night Out events
  • Complete/revise/update FSP for individuals enrolled in the Family Support Services Program
  • Complete Allocation Requests and Check Requests for individual/family needs in the Family Support Services Program and Community Outreach Program
  • Work with services providers and independent contractors to assure expenditures are within budgetary guidelines
  • Develop and maintain case files
Starting Salary and Benefits
$21.90 – $23.54 / hourly
or $22.95 – $24.59 / hour for Bilingual Case Managers

Requirements
  • An Associate’s Degree or equivalent experience in human services related field
    • OR five (5) years experience in the field of developmental disabilities, or combination of education and experience appropriate to the requirements of the position
  • Excellent oral and written communication skills; can effectively communicate with individuals served, families, staff and others
  • High degree of accuracy with time sensitive, detailed information
  • Ability to meet and maintain our driving requirements; no suspensions and less than three (3) tickets in three years
  • Valid Colorado motor vehicle license and proof of motor vehicle insurance
Preferred
  • Bachelor’s Degree
  • Two (2) years experience in service coordination/case management at Developmental Pathways or similar agency
  • Working knowledge of the DD system, rules and regulations
Additional Requirements for Bilingual Family Support Services Coordinators
  • Able to speak, write and interpret fluently in English and Spanish
  • Ability to complete all typical case management responsibilities in Spanish
  • Support with written translation of internal documents for use by stakeholders, as appropriate
