Job Description
Initial & Ongoing interdisciplinary assessment of the client
Development and evaluation of the plan of care in partnership with the client and or representative or caregiver(s)
Use sound clinical judgement in performing the prescribed treatment and evaluates the client’s response to treatment/care
Client, caregiver and family education and supervising of such education
Preparing clinical notes
Communication with the physician who is responsible for the home health plan of care and other health care practitioners (as appropriate) related to the current plan of care
Participation in the HHA’s Quality Assurance and Performance Improvement Plan
Participation in the HHA sponsored in-service training
Supervise and instruct Aide services in the client’s home and document
Maintain a safe, orderly work areas and practices
Responsibilities
Licensure: License and current registration to practice as a registered professional nurse in New York State required. Valid driver’s license or NYS Non-Driver photo ID card, may be required as determined by operational/regional needs.
