DWC has recently launched the
Every Woman Housed Initiative to mobilize housing solutions for unaccompanied women and women with children residing without shelter in the Skid Row neighborhood of Los Angeles. The key components of this initiative are outreach and intake, short-term housing responses, and long-term housing solutions.
DWC is seeking a full time Intake Coordinator to provide comprehensive, trauma-informed care to unaccompanied women and women with children experiencing homelessness. The Intake Coordinator is responsible for enrolling women in the
Every Woman Housed program, assisting women in obtaining identifying documents, linking them to needed services and resources, and collaborating across departments to meet the immediate and long-term needs of each woman. The Intake Coordinator is a member of the Clinical Department and reports directly to the Outreach & Engagement Supervisor.
- Perform intake activities for DWC’s Every Woman Housed Initiative, including field-based outreach when needed.
- Enroll women in the Homeless Management Information System (HMIS) database and conduct the Coordinated Entry System (CES) survey as needed
- Provide outreach and engagement to unsheltered women and women with children and help them connect to services
- Serve as an advocate and liaison for clients in accessing needed resources (benefits, basic needs, housing, etc.)
- Participate in developing service plans and attend multidisciplinary team meetings
- Attend weekly team meetings and regular one-on-one supervision
- Provide weekly, monthly, and as needed written reports to Supervisor as assigned
- Maintain accurate and timely record of activities and services provided to clients, case management notes, and other measurement tools
- Provide case management support as needed
- Track outcomes and monitor progress towards program goals
- Serve as an ambassador of DWC and its mission and values by participating in community meetings/events
- Develop and maintain relationships with clients, staff and local service providers
- Other duties as assigned to support the mission of the organization

This job operates in a professional office environment. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines.

While performing the duties of this job, the employee is regularly required to talk and hear. Specific vision abilities required by this job include close vision and ability to adjust focus. This would require the ability to lift files, open filing cabinets and bend or stand on a stool as necessary.
Position Type and Expected Hours of Work
This is a full-time non-exempt position. Days and hours of work are Monday- Friday 8:30 AM- 5:00 PM;
Regular travel throughout Los Angeles County is required in this position.
Option for hybrid work schedule, but with expectation to be able to work onsite full time when needed.
Required Education and Experience
- Education in Human Services, Social Work or related field preferred
- Experience with HMIS and CES database operation preferred
- Experience with intakes, assessments and data reporting
- 2 years’ experience working as a Case Manager/similar position and knowledge of community resources
- Comfort working with issues related to homelessness, substance abuse, mental health diagnosis, and history of trauma preferred
- Experience providing trauma-informed care preferred
Required Knowledge/Skills/Position Qualifications
- Strong knowledge of the complexity of poverty and women’s homelessness, mental illness, trauma, substance abuse, and health issues
- Ability to streamline intake and assessments efficiently
- Able to solve problems by being resourceful and using creativity
- Strong interpersonal, written and oral communication skills; proven ability to communicate with diverse audiences
- Ability to manage multiple tasks and priorities, including proficiency in balancing direct service responsibilities with effective record keeping
- Ability to work independently and as part of a team
- Oral and written fluency in English required; Spanish a plus
- Proficiency in Microsoft Office Suite (Word, Excel, Outlook) required
- Current valid California state driver’s license, insurance, and vehicle with good driving record
- Must be a CA resident and live at least 75 miles from the office and must be available for in-person meetings at the DWC offices.
- Must provide documentation of a negative COVID-19 test result from the last seven days, on first day of hire.
- Must provide documentation of a negative TB test result within the last twelve months, on first day of hire.
- You will be required to have received at least the first dose of COVID-19 vaccination to start employment with DWC, and second dose must be received within 30 days from the first day of hire.
- Background clearance required
