Lead Social Worker – Case Management
The Lead Social Worker functions as the subject matter expert in how to perform a comprehensive assessment of psychosocial and medical needs to the social workers on the Team. Will be responsible for reviewing NATE Tempo for discharge barriers and collaborate with SW/CM teams. Assists team to develop a plan of care to include identified clinical, psychosocial and discharge needs, coordinates plan of care; plan is documented in medical record; plan is communicated to appropriate clinical disciplines for patients with ELOS as directed by Director/Manager of Case Management. Works with the social work team for education, accountability, elevation of professionalism in partnership with Director and Manager of Case Management. Participates in the interdisciplinary teams at the facility as needed to offer guidance, suggestions and leads by example. Teaches social work team how to act as a liaison through effective and professional communications between and with physicians, patient/family, hospital staff and outside agencies.
- Masters degree in Social Work (MSW) required
- LCSW preferred
- Case Management certification preferred
- Minimum of 5 years experience in a healthcare setting required
Basic social worker skills including but not limited to empathy, sensitivity to cultural and ethnic diversity, advocacy, communication and crisis intervention
