LPN/RN Continuum Care Coordinator

A nursing home group is seeking an LPN/RN as a continuum Care Coordinator.

Job Description:

Deliver high quality clinical status review, Medication reconciliation, consulting nursing care to adult and geriatric patients post discharge from SNF setting.

• Effectively communicate with patients’ physicians and broader care team.
• Ensure that patient is receiving the appropriate care guided by physician’s care plan and your experience.
• Provide ongoing education to help patient better manage their health.
• Document all communication and coordination of patient care.

Qualifications and Skills

• LPN or RN license
• Positive and friendly attitude
• Excellent active listening, written, verbal communication skills
• Demonstrate excellent organizational and time management skills
• Provide compassionate care
• Possess problem solving skills.

Flexible hours

Salary Range: Based on experience
