Job Title: Family Care Coordinator

Reports to: Practice Manager, MA Float Lead, Medical Director

Secondary Supervision: Administration Manager, MA Leads

Status: At will, non-exempt employee

The long-term goals of the position are to improve patient lives, health, and wellbeing by expanding Family Medicine to include Mental/Behavioral health and Social care across individuals, families, and communities. Also, the goal is to support Prevention and Planning services in the practice. The position has the following primary roles to serve WMC and the local community:

  • To participate on a Clinical Care Team as an integral member in direct and indirect patient care in all areas of a clinic with a multidisciplinary team,
  • To participate in practice-based research projects at Colorado Center for Primary Care Innovation, the University of Colorado Department of Family Medicine, and Centers for Medicare and Medicaid (as needed).

Summary of Duties


  • Supports the Family Program Care Manager to implement the WMC Care Management Policy & Protocol of Activities for High-Risk Patients. Calls and schedules visits to support Clinical Quality Measures workflows, including – Prevention, Chronic care, Maternal health and family planning.
  • Answers/receives calls for incoming patient needs.
  • Works with assigned Providers for scheduled and unscheduled coverage. Conducts patient vitals and rooming, pre-or-post visits for screening and/or prevention, and WMC Medical Assistant workflows.
  • Additional shared responsibilities and duties to evolve over time.


  • Works at the Front Desk, and implements workflows accordingly on Float MA Team.
  • Additional shared responsibilities and duties to evolve over time.

Education and Experience

  • Required – LPN license or MA certification

Will consider graduation from an accredited university with a background in science, including a BA or BS in Biology, Chemistry, Sports Medicine, Public Health, Health Education, Nursing, Anatomy & Physiology, Behavior Science, Social work, Psychology, or a similar degree


  • 3-5 years experience as a Medical Assistant in Family Medicine, Pediatrics, or Internal Medicine; experience in hospital settings will be considered, especially with chronic disease management or care management experience
  • 3-5 experience with Athena (preferred EHR)
  • LSRT certification (preferred)
  • Training in motivational interviewing (preferred)
  • Very flexible (on the MA Float Team), self-disciplined, energetic, passionate, creativity, innovation, experimental, and leadership qualities showcased daily
  • A team player that can follow a regimented system and protocol to achieve a common goal
  • Highly organized and well-developed oral and written communication skills
  • Demonstrates sound judgment, decision-making, and problem-solving skills
  • Able to maintain confidentiality with all aspects of information in accordance with practice, State, and Federal regulations
  • Confidence to communication and outreach to other community health care organizations and personnel

Job Type: Full-time

Pay: $18.00 – $24.00 per hour
